Appointments with our Physiatrist are available by referral from your Family Physician and are covered by OHIP with a valid Ontario health card.
Once we receive your referral, our team will contact you to schedule your appointment.
If you do not have a referral, our friendly staff will provide you with a Sports Medicine Intake Form to complete. Upon completion our team will contact you to schedule your appointment.
